      <title>Fundraisers Help Make Memories for Dementia Patients</title>
      <description>&lt;p style="text-align: justify" class="MsoBodyText"&gt;&lt;strong&gt;Caring fundraisers in North East Lincolnshire are helping to bring the past to life for people living with dementia and memory loss.&lt;/strong&gt;&lt;/p&gt;  &lt;p style="text-align: justify" class="MsoBodyText"&gt;Local group The Ladybirds have collected a fantastic &amp;pound;1,000 through coffee mornings, dances, bridge events and a jumble sale as well as money raised by customers of Lilly&amp;rsquo;s Tea Rooms, Laceby, through their Christmas Fayre and raffle.&lt;/p&gt;  &lt;p style="text-align: justify" class="MsoBodyText"&gt;The money will help North East Lincolnshire Care Trust Plus (NELCTP) purchase a Reminiscence Pod. This is a &amp;ldquo;Pop Up&amp;rdquo; 1950s style living room which can be used in settings such as hospitals and care homes to help people with dementia feel more at ease in these unfamiliar locations.&lt;/p&gt;  &lt;p style="text-align: justify" class="MsoNormal"&gt;&amp;ldquo;This is a fantastic sum of money raised by the Ladybirds,&amp;rdquo; said Jeanette Logan, Service Lead for Older People with NELCTP. &amp;ldquo;Purchasing the Reminiscence Pod will make a huge difference to many people living with conditions such as dementia in North East Lincolnshire.&lt;/p&gt;  &lt;p style="text-align: justify" class="MsoNormal"&gt;&amp;ldquo;The Pod is easily transportable and will provide a complete environment with things to see, touch and interact with, to stimulate an individual&amp;rsquo;s memory. It will help people living with dementia communicate about their lives and experience something familiar and safe at a time when often nothing makes sense to them.&amp;rdquo;&lt;/p&gt;  &lt;p style="text-align: justify" class="MsoBodyText"&gt;Dementia affects the lives of around 700,000 people in the UK.       <title>Patients can access health and social care website in GP Practices</title>
      <description>Patients can now access a one stop information and advice website for all adult social care, health care and community support services available in North East Lincolnshire, at their local GP Surgery.&amp;nbsp;&lt;br /&gt;&lt;br /&gt;The services4me website was launched in four primary care practices in North East Lincolnshire during January, as part of a pilot project to offer members of the public easy access to information and advice about local care and support services.&amp;nbsp;&lt;br /&gt;&lt;br /&gt;&lt;u&gt;www.services4.me.uk&lt;/u&gt; &amp;nbsp;is accessible to everyone - individuals using adult social care or health care services, self-funders of care, carers, members of the public, health and social care professionals and providers of services can all make use of the resources available on the website.&amp;nbsp;&lt;br /&gt;&lt;br /&gt;Services4me touch screen computers and kiosks are now available for patient use in Freshney Green Primary Care Practice, The Raj Medical Centre, Pilgrim &amp;amp; Roxton Primary Care Practice and The Chantry Health Group Practice. Patients can now self-access information and advice on housing options, how to keep healthy, home care services, transport and leisure, benefits, caring, work, learning and links to national sources of help and support. Local residents can also search for support groups, clubs, activities and events happening in the local area.&amp;nbsp;Services4me surgeries in these practices, will be run by community volunteers to help patients with information searching and demonstrate how to use the website. &lt;p style="margin: 0cm 0cm 0pt; text-align: justify"&gt;&amp;nbsp;&lt;/p&gt;Dr Nathalie Dukes, Freshney Green Primary Care Practice said;&lt;em&gt;&amp;ldquo;We welcome the introduction of Services4me in Freshney Green. The website is long overdue for our area, both patients and professionals can now find out what help and services are available in North East Lincolnshire on a wide range of health and social care related subjects. Services4me empowers individuals to take more control over their own lives.&amp;rdquo;&amp;nbsp;&amp;nbsp;&amp;nbsp;&lt;br /&gt;&lt;/em&gt;&lt;p style="margin: 0cm 0cm 0pt; text-align: justify"&gt;&amp;nbsp;&lt;/p&gt;Carol Ranyard, Information Officer, The Pilgrim &amp;amp; Roxton Practice said; &lt;em&gt;&amp;ldquo;The Roxton Practice and Pilgrim Primary Care Centre users have close links with Community and host many of the outside Health Agencies within the waiting room.&amp;rdquo; &lt;/em&gt;&lt;em&gt;&amp;nbsp;&lt;/em&gt;&lt;em&gt;&amp;ldquo;Our Patient Group are very active within the waiting room and are currently helping patients use the new Services4Me.&amp;nbsp;&amp;nbsp; We were pleased to welcome the Mayor of Immingham, Councillor Michael Burton, to the launch of this new service. I see the new Services4Me as a great asset to our patients and other users of the Health Centre as the information is instantly available, carers and family members can also access this service on behalf of relatives.&amp;rdquo;&lt;/em&gt;&amp;nbsp;&lt;br /&gt;&lt;br /&gt;Services4.me.uk is still under development to offer local residents many more exciting and useful facilities.&amp;nbsp; Individuals who may need help&lt;strong&gt; &lt;/strong&gt;with social care needs, will soon be able to complete an on-line self-assessment.&amp;nbsp; By registering on the website in a secure way, members of the public will be able to create their own user account and answer a few basic questions about the type of help they need, which is sent to A3 Service Advice Officers who will let individuals know whether they should go on to complete a full on-line self-assessment of their needs.       <title>A "burning" need for action</title>
      <description>&amp;nbsp;&lt;u&gt;&lt;strong&gt;&amp;nbsp;&lt;/strong&gt;&lt;/u&gt;&lt;strong&gt;It is estimated that there are around 37,000 adult smokers in North East Lincolnshire who between them smoke half a million cigarettes every day. With smoking being by far the greatest cause of preventable ill health and premature death in England, officers at North East Lincolnshire Council and the Care Trust Plus are concerned about the large number of local residents suffering from smoking related health conditions.&lt;/strong&gt;&amp;nbsp;&lt;br /&gt;&lt;br /&gt;Trevor Parkin, tobacco control alliance coordinator for Care Trust Plus and North East Lincolnshire Council said, &amp;quot;Our area has one of the highest smoking rates in the country and in some of our least affluent wards, almost half of adults smoke. If the statistics stay the way they are at the moment, as many as one in seven of all adults living in North East Lincolnshire will die from a smoking related illness and that increases to almost one in four adults in some wards. Many smokers are losing up to 16 years of life due to smoking.&amp;rdquo;&amp;nbsp;&lt;br /&gt;&lt;br /&gt;&amp;ldquo;Despite all of the health messages that are out there, far too many young people, particularly girls, are taking up smoking and almost one in four women in the area smoke during pregnancy which is putting at risk both their health and that of their unborn child. We know however that two out of three smokers say that they want to quit and we need to find more ways of supporting them to do this&amp;rdquo;&amp;nbsp;&lt;br /&gt;&lt;br /&gt;An event is being held on Friday, next week for elected members, officers, health professionals, community organisations and representatives from partner organisations to hear about action being taken locally and look at&amp;nbsp; ways of enabling more adults to quit successfully, preventing fewer young people from starting to smoke, reducing the availability of cheap and illicit tobacco and much more.&amp;nbsp;&lt;br /&gt;&lt;br /&gt;Councillor Mick Burnett, deputy leader and portfolio holder for health, well-being, culture and tourism said, &amp;ldquo;It is a priority for North East Lincolnshire Council to improve the health and well-being of everyone and reducing smoking among our citizens is key to this. Local admissions to hospital and deaths from smoking related diseases are between 30 and 50 per cent higher than the national average.
