Who we are and what we do

North East Lincolnshire Care Trust Plus is a new organisation with responsibility for the commissioning of health and adult social care services in North East Lincolnshire. It also manages the community health and adult social care staff who provide these services to local patients and service users.

The new organisation replaces the former North East Lincolnshire Primary Care Trust in full, as well as inheriting responsibility for the management of adult social care from North East Lincolnshire Council.

The Care Trust Plus is the first of its type in the country and allows a much greater degree of integration between health and social care. It also provides much greater opportunity for the co-ordination of action plans to tackle the wider determinants of health and wellbeing, such as education, employment, housing and lifestyle.

The Care Trust Plus is responsible for a combined health and social care budget of £270M  for the 170,000 residents of the Immingham, Grimsby and Cleethorpes area. We fund the NHS services provided by your local hospital in Grimsby, mental health services and adult social care as well as more specialist services provided by hospitals outside of the immediate area and independent providers of social care.  The Care Trust Plus also directly provides services such as health visiting, district nursing and learning disability.

A major feature of the Care Trust Plus’s responsibilities is the work it undertakes in partnership with GPs (family doctors), Dentists, Pharmacists and Opticians – known collectively as Primary Care Practitioners. With them we plan the provision of local NHS services to meet the needs of you and your family.
