Our Mission

'To help improve the health and wellbeing of the people of North East Lincolnshire.'

Vision/values

The local shared vision is of a high performing inclusive and integrated approach to the delivery of services and commissioning wherever these are required, and which are sensitive to the needs of individual communities. To achieve this vision NEL CTP has identified 4 key themes which describe the overall goals of the organisation and its partners.

  • Creating a healthier community
  • Accessible, responsive and quality care
  • People in control of their own care 
  • A social and financially sustainable care system
Each theme has 3 component areas, linked to the CTP’s internal performance framework and a number of actions, which are detailed in the various sections of this plan.

Following the guidance of our three themes, in developing North East Lincolnshire Care Trust ‘Plus’ we intend to have: ·

  • An efficient, joined up community health and adult social care organisation that significantly improves the delivery of individualised care to the residents of  North East Lincolnshire and better reflects their needs;
  • The bringing together of health improvement and wider health and well being functions to enable more rapid and effective tackling of health improvement and health inequalities;
  • An organisation that works as part of the Children’s Trust arrangements to   assist with and ensure improved outcomes for children and young people, especially as they move to adulthood;
  • Commissioning services in a way that meets the health and social care needs of people in North East Lincolnshire and also meets the needs of individual neighbourhoods. This approach will also enable the alignment of direct           payments/individualised budgets with GP practice based models, which in turn assist increased neighbourhood governance and accountability;
  • Arrangements in place that facilitate the continued development of a strong commissioning organisation that aspires to achieve world class commissioning status;
  • Development of community governance and accountability arrangements that puts the public at the centre of our vision;
  • Strategic and specialist commissioning with North East Lincolnshire Council, North Lincolnshire Primary Care Trust and with regional/sub regional specialist  commissioning groups and others where services need to be planned and commissioned over a larger area because they are more specialist or affect a small number of people;
  • The establishment of four commissioning groups focused on delivering the health and social care commissioning priorities for their constituent populations through strong practice based commissioning; commissioning groups are the conduit for the voice of the local community and for ensuring accountability to the local community through community governance arrangements. 

CTP Values

In the lead up to the creation of the Care Trust Plus, a series of workshops were held involving staff and managers from the Community Care directorate within North East Lincolnshire Council, the PCT and Primary Care. The values established at that point were:

  • Working in partnership: being open and honest in all that we do; involving and empowering staff, local people, local statutory organisations, community and voluntary organisations, service users and carers in reviewing and designing services to meet the needs of local people.
  • Service Excellence: strive to deliver high quality services, responsive to changing needs, enabling real choice, respecting the diversity of individuals and communities within North East Lincolnshire and the diverse range of influences on an individual’s health and wellbeing.
  • Teamwork: being accountable and supportive to each other as staff and volunteers appreciating our differences, developing our teamwork by empowerment and learning and innovatively exploring new ideas together. 

A year in to the Care Trust Plus these values were revisited, also taking account of the NHS values within the draft NHS Constitution:

  • Respect and dignity;
  • Commitment to quality of care;
  • Compassion;
  • Improving lives;
  • Working together for patients;
  • Everyone counts

A key focus has been to not lose the sense of what staff felt was important. Members of the management team and the Board have drafted the following set of values based on those above. Staff and our partners in Primary care are now engaged in determining whether these values resonate with the ethos of the Care Trust Plus.
