Most people tells us they prefer to spend as much time as possible living in their own homes or as near to their own community as they can.
Working closely with its partners, the CCG aims to support people living with disability to live as independently as possible. This means reviewing current services to make sure the person is always at the centre and reviewing people living out of area to give them the opportunity to move back to North East Lincolnshire.
The CCG aims to move commissioning further towards mental health awareness and placing more emphasis on prevention and helping people maintain good mental health and wellbeing. Within primary care there will be a greater focus on pathways and partnership working with NAVIGO as well as ensuring providers are able to respond to the long term care needs of service users.
