www.services4.me.uk offers a universal information and advice point for adult social care, health care and voluntary and community sector services available in North East Lincolnshire. The website is being developed as part of a partnership between the Care Trust Plus, North East Lincolnshire Council and Voluntary Action North East Lincolnshire.
www.services4.me.uk is designed for everyone living or working in North East Lincolnshire, including those accessing adult social care or health care services, self-funders of care, carers and those who simply want to find out more about the range of support and wellbeing services available in our local community.
Health and social care professionals are making use of the website to assist with information provision and signposting the public to services that match their needs. Care and advice workers, including the CTP A3 Single Point of Access Team, voluntary agencies, frontline staff, GP’s and service providers, all have access to a consistent view of the information available.
The website enables you to search for information under different subject categories with national and local website links for further advice.
A second phase of development is now taking place up to the end of 2011 to include:
- What’s On - calendar of local health and social care events, community activities, meeting groups and clubs.
- Write a review of services used facility with the ability to view star ratings of services.
- An e-marketplace facility for the public to purchase help and support related products and services, from browsing through the directory
- A personal registration facility for individuals to hold their own account, with links to CTP case management, allowing service consumers to complete an on-line self-assessment, view and update personal care plans and use a messaging service to connect to other on-line users.
