Role and Purpose of the Community Learning Disability Team
The Community Learning Disability Team is part of the Care Trust Plus. We are a multidisciplinary team, comprised of Community Nurses, Social Workers, Community Learning Disability Workers, Carers’ Support Workers, Admin, Advice Officer, Health & Wellbeing Co-ordinators, Senior Clinical Medical Officer, Speech Therapist, Physiotherapists and Psychologist.
We commission and provide a service to adults aged 18+, who have a learning disability and are living within their local community, in accordance with the NHS & Community Care Act 1990, Health of the Nation and Patient Charter Legislation. We also assess the needs of young people during transition from children’s to adult services.
Team Philosophy
We will provide a pro-active service, which addresses and promotes the health, social, educational and cultural needs of people with a learning disability, enabling them to live ordinary, self-determined lives within their local community.
We will promote the key objectives of Valuing People, ie, Rights, Independence, Choice and Inclusion.
We will promote positive options for people with a learning disability.
What We Provide
The Community Learning Disability Team offers a confidential service for individuals with a learning disability, their parents/carers and statutory, voluntary or private agencies. We assess people’s needs before arranging services to meet those needs.

Care Management
The Community Learning Disability Team assesses individual needs, develops Care Plans and purchases services for adults with a learning disability and their carers. The team reviews and monitors these services in liaison with service users, their carers and providers.
