Transforming Community Services

What is it and what does it mean for the CTP?

The national Transforming Community Services (TCS) programme aims to improve community services to deliver modern personalised and responsive care of a consistently high standard. It is a change programme for the delivery of services to help meet the aspirations of the NHS Next Stage Review.

Within Care Trust Plus the work on Transforming Community Services is being developed and supported by Peter Noble, Project Director - Provider Services, working closely with Lance Gardner, Director of the Care Directorate, and Kevin Bond, Director for Mental Health. There are 7 projects being progressed in the work programme. These are Board Development, Mental Health Strategy, Care Strategy, Commissioning Engagement, Information, Performance Management and, Business Support and Care Plus Strategy. The work is being undertaken in conjunction with primary care and clinicians with the Provider Board overseeing the overall programme.

By October 2009 both the mental health and care directorates need to be in a state of ‘business readiness’ as providers. Between October and April the transformational plan of work will begin to be implemented. By April 2010 there will be a 5 year plan for both Directorates and an Estates Strategy for the CTP.

The importance of the overall programme is that staff at all levels have a clear understanding on how they inform and contribute to high impact changes in service provision that have a tangible benefit to the local community. To support these arrangements it is intended to convene wider workforce events over the coming weeks. We will also be substantially developing this site to support communications.
