Transforming Care is led by a vision to radically change the way our health and social care services are designed and delivered, to people needing care and support, their carers and families.

The Care Trust Plus is a health and social care body that is committed to Transforming Care in North East Lincolnshire, to enable people, even those most disabled or disadvantaged, to participate as active citizens in their community and to take as much control as possible over their own lives. 

To achieve this we are working very closely in partnership with members of the Public, the Council, our staff and volunteers, the voluntary and private sectors and most importantly, by listening to people who need care and support and acting on their views.

All parts of England are working towards the Transformation of Care, with the aim of giving people who need health and social care service much more choice, control and flexibility to design and buy their own care packages, to meet their  personal needs and circumstances.

In the future, everyone eligible for Care Trust Plus funded help with their social care, will be offered a sum of money, called a Personal Budget to spend on meeting their support needs. In turn we will also extend this to health services through Personal Health Budgets.

The Care Trust Plus Transforming Care Programme has set up a number of work projects to research, develop and implement change over the next 2 years.
