Self-Directed Support
Development of the new Self-Directed Support system of care and offering Personal Budgets to everyone eligible adult social care funded support. Self-Directed Support enables you to have more choice and control over the type of care you want and how it is provided to you.
Personal Health Budgets
Research and development of approaches to personalise health care services and considering the range of options available to you, from enabling choice, control and flexibility over your support needs and preferences, to the use of Personal Health Budgets for health care.
Communications
Delivery of a marketing communications strategy to raise awareness and understanding of Transforming, using a range of social marketing approaches and providing a range of communication materials in different formats, to suit the information needs of different people including those using services, carers, general public, our staff and health professionals.
Services4.me.uk Information & Advice Website
www.services4.me.uk offers high quality, user friendly information and advice directory about all adult social care, health care and third sector services available in North East Lincolnshire, which will all be available to everyone and accessible in one place. The website will also enable you to rate services, see star ratings of services, search for information in different ways, download leaflets about health and social care topics and join a forum to chat to others.
Community Engagement
Providing a community wide membership scheme called ‘Accord’, to enable you to have a direct say in how health and social care support services are developed in North East Lincolnshire and enable you to influence local decision making on issues that affect your daily life.
Reshaping the Learning Disability Market
Creating new opportunities and choices for people with learning disabilities to be supported lead their own lives, in their own homes, by encouraging providers of support and housing to develop new local services.
Personal Assistant Bureau
Delivery of an Independent Personal Assistant Bureau in North East Lincolnshire, so if you need support, we can offer you real choice and availability of Personal Assistants to meet your personal needs and preferences.
Workforce Development
Developing, training and supporting Care Trust Plus employees and volunteers to make sure we deliver high quality support, personalised services and work in partnership with you, family, friends and carers.
