What is Personalisation?

Personalisation means that everyone who needs health and social care support, regardless of their level of need, whether from public services, the voluntary and community sector or from local businesses, will have greater choice, control and flexibility over how they receive their care and support. 

We want those needing care to be able to live their own lives as they wish, confident that services are of high quality, are safe and allow people to meet their own needs for independence, well-being and dignity.

A personalised care system means that you can complete your own self-assessment about what care you need, rather than a care package chosen by your social worker. You are at the centre of the care and support planning process – we call this ‘person centred planning.’  Personalisation supports the idea that you are best placed to know what support you need and how those needs may be best met.  You can take control over decisions that affect your life and also have information, advice and support to make the best decisionsover your care. 

A Personal Budget allows you to control your own care services with the help and support of technology, family, friends and the wider community.  If you are unable to manage your own budget, a carer, family member, your care manager or social worker can help, or another arrangement can be put in place.
