What is Person Centred Planning?


Person Centred planning is a way of assisting you to find out the type of  support you require and help you to achieve your wishes and aspirations fpr your life. Person Centred Planning is mainly used for those with a learning disability and puts the person at the centre of the support planning process, to help them gain the life they desire, with the help of family and friends.


The Plan includes a picture of a person's life, where they are now, where they want to be in the future and how the person, their friends, family or carers can help them reach their desired future.

 

Making a Person Centred Plan involves 3 stages:

  1. Pre-meeting
  2. Making a Plan
  3. Reviewing

Pre-meeting for Developing a Plan


To develop a Person Centred Plan, you will need to complete a request form.  Once the request is received, a Person Centred Planning Facilitator will contact you to arrange a pre-meeting to discuss what you wish to achieve, improve and change in your life, which becomes a focus for the 'Making a Plan' meeting.

Making a Plan

The plan can be developed in any form and is often drawn using pictures and words, to enable everyone to easily understand.  You can invite anyone – friends, family, carers, other professionals to attend the meeting.  During the meeting everyone is encouraged to be involved and take responsibility in helping you to achieve their needs and wishes.  A Graphic Facilitator will draw everyone’s ideas and ensures the plan is centred around what is important to you.  An action plan is developed to inform everyone of how they can support you to achieve the needs and aspirations in the plan.  The plan is then approved by by you.



Reviewing the Plan

 

The plan will be regularly reviewed at dates set by you, until all key actions within the plan have been addressed.  You then decide how then plan proceeds and areas within the plan can be added to or changed over time.
