The Self Assessment Questionnaire is simply a tick box question and answer form. It will help you to think about the different areas of your life and what level of care and support you need to achieve maximum independence.
You can complete the Self Assessment Questionnaire by yourself, or with the help and advice of others including a care manager, social worker, friends or family. Each question asks you to estimate the level of support you may need, what you find difficult and/or what you can do for yourself. It helps to look at a range of issues that may have an impact on your life. and you can record your own support needs and circumstances as you see them. The answers you provide in the Self Assessment Questionnaire enables the Care Trust Plus to work out the total amount of money you need to meet your care and support needs. What is a Support Plan?
The Support Plan is developed after completing the Self Assessment Questionnaire, financial assessment and once you have advised of the money available within your Personal Budget. The Support Plan can be completed by you, or with the help of friends, family, a social worker or care manager. The Support Plan is designed to look at the areas of care, support and wellbeing that are important to you and to decide what you would like to change to help you live the best possible quality of life. The Support Plan looks at all areas of a your life, what is working and not working and should help you identify the support you feel you need and how you want to receive that assistance. A Support Plan can be developed in many ways such as drawn pictures or diagrams, presentations, DVD’s, some people choose to talk about their ideas to someone else who writes them down. The Support Plan does not have to be complicated, it as detailed or simple as you wish. |
