Our Vision & Approach for Transforming Care

The Care Trust Plus is fully committted to delivering the Putting People First Transformation Programme and is changing the way iys health and social care services are provided, to give people more choice, control and support to live independently, at home for as long as possible. 

The Care Trust Plus recognises that real change, can only be achieved through the involvement of people who need care and support services and their carers, at each stage of Transformation.

The Care Trust Plus is working closely with all its key stakeholders - those people who are involved in  Transforming Care including people accessing care services, carers, the General Public, staff and volunteers, care providers, voluntary organisations to develop local services that meet the requirements of the people of North East Lincolnshire.

The Care Trust plus worked with local people accessing care and support services to develop a local vision for the future.

What Does this Mean?

  • The needs, wants and aspirations of people will come first - all of our work will be led by what local people tell us is important to them and we will listen to the needs of local people to make sure we can provide the best care and support services.
  • People will have more choice, control and freedom to shape and buy their own care and support services.  People will not have to fit in with existing services, they can design their own personal care package with help from family, friends or the Care Trust Plus.
  • Local services will be of a high quality to improve people’s experience of care and we will continuously measure and check how effective services are in improving people’s health and well being.
The Care Trust Plus is focussing on 4 key areas of Transformation to achieve the right results for people, ensure care and support services become more personalised to people's needs and wants and improve their quality of life.

What will this Deliver for the People of North East Lincolnshire?

With your Co-operation, our vision and approach to Transforming Care seeks to:
 
  • Create a ‘no wrong door’ policy so that if an organisation or person that you first contact does not have the information you need, they will make sure your details are given to the correct organization or person who can offer you help and support.
  • Improve people’s whole experience of care and support services and giving people the confidence that services are of high quality and safe.
  • Encourage and support people’s requirements for their personal independence, dignity and well-being.
  • Ensure local services are available to everyone such as transport, education, leisure, health, housing.  These services are important in everyone’s lives and we do not have to meet any criteria to access them.  We will work to make sure that barriers which stop people using everyday services are removed.
  • Give people who use services, carers and their families a greater voice, control and choice over planning and designing their support and care and also encouraging people to influence and be involved in developing future care services.
  • Focus on preventing illness and health problems and making sure support is available to help people who need more help at an earlier stage, to stay independent for as long as possible.  Including help to safely maintain the home, support to exercise, training to gain a job or return to work after a break.
  • Introduce a Personal Budget for everyone needing adult health and social care support. Encouraging and helping individuals to do their own self-assessment.
  • Introduce equipment in the home to help people to live independently.
  • Provide easy ways for people to access to information about care and support services and get the right advice.  Information will be provided in different formats such as leaflets, posters, the Internet and available at a variety of locations including libraries, GP surgeries, the hospital, community resource centres.
  • Provide better support, training and opportunities for carers.
  • Create new roles for health and social care workers to support the new system of care and increasing staff knowledge of health conditions and problems by providing more training.
