There are 4 main areas of the Transforming Care system that the Care Trust Plus will be focusing on, in partnership with people needing social care support, the local community, General Public, the Council, staff, the voluntary, public and private sectors, to make sure care and support services become more personal to people’s own needs and preferences.
The 4 areas are: Universal Services, Early Intervention and Prevention, Choice and Control and Social Capital. All of these areas need to be in place to achieve the right results for people and improve their quality of life.

Universal Services are our general support services, available to everyone locally such as transport, leisure, health, education, housing, community safety and good access to information and advice. Everyone should be able to access this support and help easily, to maintain their health and wellbeing, make choices over their life and be active members of their community.
Early Intervention and Prevention means support is available for people who need a little more help at an early stage, to stay as independent for as long as possible, such as support to recover from a long term illness, help to gain training or return to work following a break. By using assistive technologies such as telecare, people with Dementia can stay at home with their families and continue their everyday life.
Choice and Control is about enabling people to choose who delivers their care, how, where and when their care and support is provided. This also includes providing the right information and advice to people, to help make decisions about the services available. People need to be confident they are safe, reliable and suitable to buy and also have access to advocates should they need help to express their support needs.
Social Capital is about development of the local community, enabling people to experience the support, care and friendships that come from relationships with others. Also providing opportunities to engage and involve people to become full members of a community, where they can have a voice and influence decisions that affect their life.
