Who can Help?

There are a range of organisations and people that can help you:

1. Social Worker/Care Manager

A Social Worker/Care Manager can help to:

  • Complete your Self-Assessment Questionnaire
  • Put together a personal Support Plan and budget
  • Arrange for Care Trust Plus services to be provided
  • Review your care and support
  • Find answers to your questions


2. Support Brokers

A Support Broker is someone who is trained to help you look at, choose and set-up your package of support.


3. Advocates

An Advocate is someone who is independent, can help you say what you want, secure your rights, represent your interests and gain the services you need.


4. Voluntary and Community Organisations

In North East Lincolnshire there are many Voluntary and Community groups that support people with disabilities, older people and those with a mental health condition, by giving advice and information as well as practical, day-to-day or one-off assistance.


5. Friends and family may also be able to help

You can involve friends and family as much or as little as you wish, in planning and buying your support and they can also be involved in providing your care and support.
